Observed with omni-coder persistent session: after approval-style prompts (e.g. 'okay, go ahead'), the model repeatedly responds with commitments like 'On it, I will implement' but often performs no tool calls in that turn. This creates apparent stalling. Need mitigation in runtime/prompting: detect no-tool commitment responses and auto-steer (or tighten system prompt) to execute immediately when user intent is implementation. Include reproducible transcript and validation criteria.
