When the reviewer issues REQUEST_CHANGES, it should add a comment to the task with the specific feedback. This comment will then be included in the coder's prompt on the next iteration via the 'Previous Feedback' section. Currently the feedback is lost between iterations - the coder just sees the same task description and doesn't know what to fix.
