Add 'jr fact add <fact> --files=<paths>' and 'jr fact list [--files=<paths>]' commands to Jr.hs. This allows agents and humans to record facts. The 'add' creates a fact with confidence 1.0. The 'list' command shows facts optionally filtered by related files.
