Add facts table to SQLite schema

Create Omni/Jr/Fact.hs with SQLite schema for storing facts. Schema from _/llm/jr.md: facts(id, project, fact, related_files JSON, source_task, confidence REAL, created_at). Add initFactDb function and basic CRUD operations: createFact, getFacts, getFactsForFiles, updateConfidence.
