Add human notes field for intervention tasks

Allow human to add notes/guidance when investigating a stuck task:

• Add 'notes' column to tasks or retry_context
• Text area on task detail page
• POST endpoint to save notes
• Notes visible to worker when task is retried (include in prompt)
