Add design_docs table to SQLite schema

Create design_docs(id, title, goal, solution, plan, criteria, created_at, updated_at). Add foreign key from tasks to design_docs (taskDesignDoc :: Maybe Text). Design docs are markdown content stored in DB, not files.
