Route failed tasks to planner before human escalation

Modify Jr.hs autoReview to call planner when task fails 3 times BEFORE marking needs_human. The planner reviews the task, rejection reasons, and KB facts, then either: 1) replans into smaller subtasks, 2) marks as blocked with explanation, or 3) escalates to needs_human. Add NeedsPlanner status or use a queue.
